End of Life Series is a three-part presentation with Dr. Dial, Medical Director at Livingston Memorial, and Carrie Anderson, Director of Community Palliative Care at Livingston Memorial. You can sign up for all the presentations or each one individually.

Presentation 2: Advanced Directives /POLST (Physician Orders for Life Sustaining Treatment), In this class, we will be reviewing the POLST form.
“What is the POLST?”
The Physician Orders for Life Sustaining Treatment (POLST) is a medical order that helps give people with serious illness more control over their care during a medical emergency. The POLST can help make sure you get the care you want, and also protect you from getting medical treatment you DO NOT want. The POLST was developed by the Coalition for Compassionate Care. The Coalition for Compassionate Care of California is a statewide partnership of regional and statewide organizations and individuals dedicated to the advancement of palliative medicine and end-of-life care in California. The POLST is approved by the California Emergency Medical Services Authority (EMSA). - Thursday, November 14th, from 2:30pm-4:00pm.
Presentation 3: In this class, we will be reviewing the Advanced Directive called Five-Wishes. This will include detailed discussions about end of life care and hospice care. If you do not already have an advanced directive, you can take a Five-Wishes booklet home and completed it with one of the following: (1) two eligible witnesses, or (2) you can have it notarized.
“What is the 5-Wishes?”
Five Wishes is a legal document that helps adults document their wishes for end-of-life care, regardless of health or age. It’s written in everyday language and covers a range of topics, including medical and legal issues, comfort, dignity, and personal, family and spiritual concerns. Five Wishes also allows people to name a healthcare agent to make decisions on their behalf if they are unable to do so themselves. – Thursday, December 5th, from 2:30pm-4:00pm.
